    • For staff and physicians who access online courses through the Learning Management System (LMS).
  • Attending Practioner Report (PDF)
    • If you are absent from work due to injury or illness for three or more consecutive shifts, you can print the Attending Practitioner Report (APR) and take the form to your treating practitioner for completion. Please refer to the Illness, Injury and Abilities Policy on the intranet for further instructions or contact Occupational Health and Abilities with any questions.
  • Travel/Contagious Illness Reporting Form
    • It's the time of year that we see an increase in communicable illnesses such as colds, influenza and gastroenteritis. If you are experiencing this type of illness and/or have symptoms of fever, runny nose, sore throat, cough, vomiting or diarrhea it is important that you speak with an Occupational Health Nurse to ensure that you are returning to work at the correct time to avoid transmission of your illness to patients, families and coworkers. Please use the attached link to report your illness and contact information so that Occupational Health can follow up with you
    • You can also call Occupational Health at 905-576-8711 ext. 33710 to report your illness. If you have to leave a message, one of the team members will call you back at the earliest opportunity.
