Why Use MyChart™?

MyChart™ is a free and secure online tool providing you and your designated family members, friends and members of your health care team access to your personal health information.

With MyChart™, you can view and manage your health information online on a computer, laptop, tablet or cell phone at any time of day.

This information includes:

  • Lab results

  • Diagnostic Imaging reports

  • Discharge summaries and clinical notes on your care or treatment

You also have the ability upload other health information to customize portal as a personal health record.

You can input your:

  • Medications

  • Allergies

  • Immunizations

  • Height and Weight

  • Mood

MyChart™ provides you with seamless access to your health information, and encourages you and your family to be engaged throughout your care journey. Having this information can help you manage your care with your health care team and can lead to better collaboration and communication.

 Sign-Up for MyChart

Signing up for MyChart™ is easy. 

With a valid email account, you can register for MyChart™ in person or online if you are 18 years of age or older.

Register in person when you have an appointment:

  1. Give your email address to the registration clerk.

  2. In 24-48 hours, you will receive an email from the MyChart™ Support Team.

  3. In this email, you will receive a link to the MyChart™ website and a Personal Identification Number (PIN).  This PIN helps to keep your health record secure.

  4. Click on the link in the email to open the MyChart™ website.

  5. Information on the MyChart™ website will tell you what to do next. You will be asked for your PIN. 

Register in person without an appointment:

  • Visit the Health Information Management Department at Lakeridge Health.

    • This Department is only at the Oshawa, Ajax-Pickering, Bowmanville and Port Perry hospitals.

  • For security reasons, you will need to show your ID (for example: your health card). You will be asked for your email address.  When you get home, follow steps two to five above.  

  • You can register over the phone by calling the Health Information Management Department at 905-576-8711 extension 4130

Register online:

  1. Visit www.lakeridgehealth.on.ca and click on the MyChart™ icon

  2. Click on the ‘Registration Request Form’ button and complete and submit the information form

  3. When your information is confirmed, an email will be sent to you.

  4. In this email, you will receive a link to the MyChart™ website and a PIN. 

  5. Click on the link to open the MyChart™ website and the information on this website will tell you what to do next. You will be asked for your PIN to complete the registration. 

Please do not share your PIN with anyone. Use your PIN to set up a username and password for your free MyChart™account at www.mychart.ca. During registration on the MyChart™ website, be sure to enter your first and last names exactly as they are written on your Ontario Health Card or hospital card.

What is MyChart™?

MyChart™ is a free, secure online tool providing patients and designated family members, friends and members of a health care team access to personal health information. Developed by Sunnybrook Health Sciences Centre, the MyChart™ has over 110,000 patient users from nearly a dozen health organizations across Ontario, including Sunnybrook Health Sciences Centre, Michael Garron Hospital, William Osler Health Systems, St. Michael’s Hospital, and Mount Sinai Hospital.

Who has access to MyChart™?

All Lakeridge Health patients can to register for MyChart™, however, only patients who have completed the registration process will have access to personal health information online through MyChart™. Registered patients can electronically grant access family members, friends, and members of their health care team to their MyChart portal. This can be done through the “Share My Records” section found in the menu bar.

Why should I use MyChart™

MyChart™ is a free tool that provides you with easy and timely access to your health information online, at any time of day. You can use MyChart™ to create, manage and enter your own personal health information, such as your mood, blood pressure, medications, immunizations, allergies and your height and weight.

MyChart™ provides you with seamless access to your health information, and having this information available can help you manage your care with your health care team and lead to better collaboration and communication between you and your health care providers.

How can I access MyChart™?

MyChart™ will be available for Lakeridge Health patients beginning March 27, 2018. The online portal is accessible from anywhere at any time through the Internet on a computer, laptop, tablet or mobile device.

Once registered, you can log in to MyChart by visiting www.mychart.ca or by clicking “Log in to MyChart” on the Lakeridge Health website here: https://lakeridgehealth.on.ca/en/patientsandvisitors/mychart.asp

What if I do not have an e-mail account, can I still sign-up for MyChart?

You cannot access MyChart™ without a valid e-mail account. You can sign up for a free email account using a variety of services (e.g. Gmail, Yahoo, iCloud, etc.).

Is there another way for me to access my personal health information and health records besides through MyChart™?

Yes, you can follow our current process of requesting their personal health information record through the Release of Information Department.

Will MyChart™ be available to patients who are under 18 years of age?

Not at this time. MyChart™ will only be available for patients who are 18 years or older. The portal will be available to patients under 18 during in a future phase of MyChart’s implementation at Lakeridge Health.

What information can I access through MyChart™?

How soon will my health records and results be available on MyChart™after my Lakeridge Health visit?

Please see the information below about how long you will need to wait to see the result on MyChart from the time the result has been finalized.

Lab Reports:

No delay – with the exception of Pathology and HIV results, which are subject to a 14 business day delay and cytogenics, which are subject to a 4 business week delay.

All Diagnostic Imaging (DI) Reports:

Subject to a 14 business day delay.

HIM Reports

No delay except for an Operative Report which is subject to a 7 business day delay.

HIM Reports with no delay include:

  • Consultation Report

  • Emergency Report - Dictated

  • History & Physical

  • Mental Health Letter

  • Progress MD DRCC

  • Preadmission Note

  • Progress Report

  • Discharge Summary

  • Transfer Note

  • Visit History

Why are some results subject to a delay, while others are not?

Extensive consultation with patients and clinical stakeholders brought to light the need to delay certain results. This is to provide doctors and health care professionals with the necessary time to meet with you to review sensitive test results.

Will patients be able to see historical data on the Portal?

No. You will only be able to see health information and records that is finalized on or after March 27, 2018, when MyChart™ becomes available at Lakeridge Health.

Is there a fee to use MyChart™?

MyChart™ is free for patients.

Do any other hospitals use MyChart™?

Sunnybrook Health Sciences Centre developed MyChart 12 years ago and it has since been adopted in a number of other hospitals across Ontario. Some of the member hospitals are: Baycrest Health Centre, Mount Sinai Hospital, The Ottawa Hospital, The University of Ottawa Heart Institute, St. Michael’s Hospital, Toronto East General Hospital and William Osler Health System.

How secure is health information in MyChart™?

MyChart™ is secured through the same encryption technologies used by the major banks for online banking.

How can I begin using MyChart™?

During your next appointment, provide your personal email address to the registration staff. A link to a secure website will be emailed to you where you will be able to register your account MyChart™.   • If you do not have an appointment at Lakeridge Health, you can visit the Health Information Management Department at the Ajax Pickering, Bowmanville, Oshawa or Port Perry hospitals.

Can I view my health information from other health care organizations that use MyChart™?

Initially when you register for MyChart™, only Lakeridge Health medical records will be shown, however, if you receive care from another Ontario hospital or care facility that offers MyChart™, you can request to have view all of your health care information in one place by contacting the MyChart™Support Team.

What happens if I lose or forget my password?

If you lose or forget your password, you can reset it by clicking the “forgot password” link on the MyChart™ login page.

I have a question about a result of mine on my MyChart portal – who should I contact?

Your doctor or nurse will discuss your test results with you at your next appointment.

If you are concerned about how correct the MyChart™ records are, please contact the Health Information Management Department at 905-576-8711 extension 4130. 

What should I do if I see health records or information on my MyChart™ portal that does not belong to me?

If you see someone else’s personal health information when you log in to MyChart™, please contact Lakeridge Health’s privacy office right away by phone at 905-576-8711 ext. 2663 or submit your concern on our website here.
