Congratulations to Lakeridge Health’s Diabetes Program for successfully completing the Diabetes Canada - Standards Recognition Program.
“This achievement validates that our patients are receiving the best possible care that we can provide,” says Heather Reid, Lakeridge Health’s Director of Regional Nephrology and Diabetes.
The Diabetes Education Programs and the Centre of Complex Diabetes Care enrolled in an extensive review to ensure alignment with at least 80% of the Diabetes Canada Outcome, Process and Structure Standards. Lakeridge Health’s Diabetes Program achieved a perfect score.
“The recognition program is also a requirement within our Central East LHIN for all Diabetes Programs, so achieving 100% of the standards reflects our strong role in the Durham Region in supporting patients with diabetes,” says Reid.
“This achievement is reflective of our team’s commitment to our mission, which is to educate, support and empower people to effectively self-manage diabetes,” says Rishita Peterson, Patient Care Manager of the Diabetes Program.
In a letter from Dr. Carol Huang and Gail MacNeill, RN, Co-Chairs of the Quality Committee for Diabetes Canada’s Standards Recognition Program, the Diabetes Program was commended “for its efforts and achievements in completing this report and in their commitment to providing the best possible diabetes self-management education and support to their patients and their community.”
Once recognized by Diabetes Canada, the program maintains status for five years.
Says Reid, “Receiving this recognition from Diabetes Canada means that we are confident that our patients are getting excellent care while receiving our services. This formal review of our program by an external organization is part of our corporate expectation to deliver on our commitment to provide population-based and evidenced-based care to our patients.”
