DUTIES & RESPONSIBILITIES:

Working as a member of the inter-professional team, the Nurse Practitioner (NP) will provide specialized comprehensive geriatric services in the Post Acute Specialty Services (PASS) program. The NP, will be the Most Responsible Practitioner (MRP) and responsible for:  admissions to and discharge from PASS CCC inpatient beds, autonomously diagnosing; ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice; working in close collaboration with the medical staff utilizing Medical Directives to ensure a timely  response to a clients' changing health status and needs; and providing direct patient care focusing on health promotion and maximizing patients' safety and function.  Actively participating in regular team meetings, liaising with community based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed.  This position is ideal for a Nurse Practitioner with a passion for geriatric care.

QUALIFICATIONS:

* Completion of a recognized Nurse Practitioner program;

* Masters of Nursing;

* Current certification with the College of Nurses of Ontario;

* Sound working knowledge of hospital nursing functions, current needs in nursing, health care and geriatric care;

* Previous experience in clinical work with geriatric clients;

* Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively;

* Effective verbal and written communication skills;

* Strong interpersonal, organizational and problem-solving skills;

* Ability to work well within an inter-professional team and to foster effective working relationships;

* Ability to exercise initiative and observe strict confidentiality

* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area; and

* Proven good attendance record with capability of maintaining this same standard.
