Scarborough Hospital to Home Program

Located at the Rouge Valley Health System (RVHS) and the Scarborough Hospital (TSH), the Hospital to Home Program (H2H) is a collaboration between Durham Mental Health Services (DMHS), Lakeridge Health (LH) - Pinewood, RVHS and TSH.

The H2H Program connects individuals who present with mental health and/or addictions concerns at RVHS and TSH (Emergency Room, Crisis Unit, Urgent Care, and Mental Health and Medicine Inpatient Units) to community supports and services.

The goal is to provide individuals with the information, education, resources and the planning needed to manage in the community, thus decreasing the frequency of visits to the hospital.

The Hospital to Home Program aims to decrease frequency of hospital visits by providing strength-based short-term community support. This program can help to encourage the individual to manage mental health and addiction issues within the community with the appropriate resources.
