Team Member COVID-19 Passive Self-Screening

  • Team members no longer use the Fast Pass app or Tap-N-Go option for screening and will be responsible for self-screening prior to EVERY shift.
  • Team members must complete screening at home prior to EVERY shift using this screening guide.
  • If a team member does not pass self-screening, they are to notify their supervisor and report to Occupational Health using the appropriate online form.
  • Team members who do not pass screening cannot report to the workplace onsite until they are cleared by Occupational Health.

COVID-19 Reporting Forms

 

Please use the forms below to report COVID-19 symptoms, a COVID-19 exposure, or a positive COVID-19 test result, and receive guidance from Occupational Health nurses.

Even if you have reported your absence to your leader, it is imperative that, if any of these situations apply, ALL team members use these forms to report to Occupational Health. Please ensure that the appropriate form is FULLY completed before it is submitted.

Employee Assistance Program (EAP) Provider, Guidance Resources

    • For staff and physicians who access online courses through the Learning Management System (LMS).
  • Attending Practioner Report (PDF)
    • If you are absent from work due to injury or illness for three or more consecutive shifts, you can print the Attending Practitioner Report (APR) and take the form to your treating practitioner for completion. Please refer to the Illness, Injury and Abilities Policy on the intranet for further instructions or contact Occupational Health and Abilities with any questions.
  • At Home COVID-19 Antigen Testing Reporting
    • Please contact Occupational Health at 905-576-8711 ext. 33710 if you have difficulty completing the form. The Internet Explorer browser is incompatible with the form, please use an different browser such as Google Chrome or Safari.
