Outpatient Palliative Care

We help patients and their families who are living at home with a serious illness. Our team is made up of Palliative Care physicians, Advanced Practice Nurses, Occupational Therapists, Spiritual Counsellors, Social Workers, Pharmacists, Physical Therapists, Dietitians, and Chaplains who work closely to ensure you receive the best possible care at every stage of your disease.

Services are available through a referral by a family physician, nurse practitioner, oncologist, or other primary health care provider or specialist.

We can help by:

  • Managing your pain and other physical and emotional symptoms like depression and anxiety,
  • facilitating community supports,
  • and providing aid in decision making and advance care planning.

At our Oshawa site, Outpatient Palliative Care services are offered in a clinic-like setting every Monday to Friday from 8:30 a.m. to 4:30 p.m. at the R.S. McLaughlin Durham Regional Cancer Centre in Oshawa.

In Bowmanville and Port Perry, Outpatient Palliative Care services are offered at individual palliative care physician offices.

Home visits may be arranged from all sites depending upon geography and availability.
