Helping Patients in Durham Region Stay Connected to Their Health Information
Durham Region — Thousands of patients and families in Durham Region are now able to have easy and secure access to their Lakeridge Health records and select health information through the launch of the online patient portal MyChart™.
Lakeridge Health introduced the electronic health record sharing system across its regional system of care earlier this month. With MyChart™, users can view and manage health information online on a computer, laptop, tablet or mobile device at any time of day. After signing up, MyChart™ users will have access to Lakeridge Health lab results, diagnostic imaging reports, and select health information reports finalized on March 27, 2018 or later.

It provides convenient access to health information and an opportunity for improved communication with health providers throughout their journey.”
In addition, patients will have access to a variety of features through MyChart™, including a personal health tracker to help manage and monitor their well-being. Phase two features of MyChart™, such as appointment viewing and education resources, are expected to be available later this year at Lakeridge Health.

Signing up for MyChart™ at Lakeridge Health
With a valid email account, Lakeridge Health patients can register for MyChart™ in person or online if they are 18 years of age or older.
In-person
Patients can register for MyChart™ at any Lakeridge Health hospital with a registration clerk if they have an appointment. If a patient doesn’t have an appointment, he or she can visit the Health Information Management Department at the Ajax Pickering, Bowmanville or Oshawa hospitals to register for MyChart™
Online
Registration is also available through Lakeridge Health’s website. On the Lakeridge Health website click on the MyChart™ icon and click on the ‘Registration Request Form’ button and complete the information form.

- Developed by Sunnybrook Health Sciences Centre, MyChart™ has over 110,000 patient users from nearly a dozen health organizations across Ontario, including Sunnybrook Health Sciences Centre, Michael Garron Hospital, William Osler Health Systems, St. Michael’s Hospital, and Mount Sinai Hospital.
- Lakeridge Health patients aged 18 years and older who register to sign up for MyChart™ will have online access to lab results, diagnostic imaging reports, and select health information reports (e.g., discharge summaries) finalized on or after March 27, 2018.
