You may request access to information at Lakeridge Health under the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA provides members of the public with a general right of access to information within our hospital’s custody or control as of Jan. 1, 2007 (with limited exceptions and exclusions).
Health records and personal health information are not subject to FIPPA. Please refer to the Lakeridge Health privacy page for information about accessing your health records/information.
Anyone can make a formal request under FIPPA by mailing or delivering a written request to Lakeridge Health’s Privacy Office at:Privacy Office – Lakeridge Health
For your request to be considered complete it must meet the following criteria:
In addition to the $5 application fee, the fees listed below may also apply to your request (as permitted by FIPPA). For each access request received, Lakeridge Health will provide you with a fee estimate. If the fees associated with your request are estimated to exceed $100, we will require a 50 per cent deposit prior to proceeding. We will always advise you of any fees up-front, before processing your request.

|Record preparation||$7.50 per 15 minutes required to prepare records for release.|
|Photocopying||20 cents per page|
|Computer programming||$15.00 per 15 minutes if needed to develop a program to retrieve information.|
|External Costs||Any other costs incurred in responding to a request for access to a record|
|Shipping||Actual costs to deliver as indicated by requestor.|
