Status: Regular Part Time (The current schedule is Days/Evenings/nights/weekends: 8 & 12 hour shifts)

DUTIES AND RESPONSIBILITIES:

The RN - Post Acute Speciality Services provides patient care within the nursing process lifecycle.  As a member of the Health Care Team, Nursing Team and as an individual practitioner, the incumbent maintains competence and professionalism relevant to current practice.  The successful incumbent will maintain nursing records, assessments, screening and scheduling of patients, monitoring patients' medical status as it impacts on participation in programming, assists in prioritizing waiting lists, statistical recording, and participation in quality improvement initiatives within a multidisciplinary team.  

QUALIFICATIONS:

*Registration with the College of Nurses of Ontario (in good standing);

*demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;

*current B.C.L.S.;

*significant current experience and evidence of recent related continuing education;

*demonstrated self motivation regarding continuing education in areas of patient assessment, patient education, leadership and related Clinical Specialties;

*excellent assessment skills with clinical knowledge of complex stroke pathophysiology as well as general neurological conditions;

*proven good organizational, prioritization, time-management, problem-solving and teaching skills;

*demonstrated excellent communication skills both written and verbal;

*strong interpersonal skills with the ability to foster good public relations and effective working relationships in a family centred environment;

*demonstrated experience with intake coordination and philosophy of adult rehabilitation;

*demonstrated ability to perform appropriately in emergency situations;

*ability to lift, turn, transfer patients manually and with mechanical aids;

*proven ability to assume a leadership/preceptorship role;

*demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety program and practices including staff, patient and environmental safety and participation in regular in-service education in this area;

*working towards certification as a Rehabilitation Registered Nurse is preferred; and

*proven good attendance record with capability of maintaining this same standard.

 

SALARY: $29.36 to $42.44 per hour
