Your Medication History
Bring Your Medications With You!
When coming to the hospital, please remember to bring all your current medications with you. We need to know all the different types of medication you are taking, the dose and frequency. This includes all prescriptions, over-the-counter medications, and herbal/vitamins.
The hospital has limited access to the prescription information kept at your community pharmacy or information about any other medications you might be taking.
For this reason we created a patient medication history form. Please print off this form and fill it out and bring it to the emergency department when you visit.
