Patient Satisfaction Survey
As part of our commitment to continuous monitoring and improvement of services, we ask that you provide us with your opinion and views about your hospital stay. You may do this in several ways.
- First, you may contact the Clinical Leader in the area of your hospital stay to discuss the care you received at Lakeridge Health.
- Secondly, you may fill out a "We Care What You Think" Survey. Click here to obtain the survey.
In addition, after being released from the hospital you may receive a survey in the mail. Lakeridge Health along with 96 other Ontario Hospitals participates in ongoing patient satisfaction surveys that are overseen by the Ontario Hospital Association. If you are randomly selected and mailed a survey, we ask that you complete the survey and return it in the self-addressed pre-paid envelope that will be provided to you. The information we receive from your responses will allow us to gauge how we are doing compared to all other participating hospitals.
